Forest Hill Hockey Association in Partnership with Canadian Tire Jumpstart

Hockey, and team sports in general, have wonderful benefits for children. They teach discipline, patience, responsibility, cooperation and teamwork; they encourage active and healthy lifestyles; and they provide children with an opportunity to have fun, take pride in their efforts and accomplishments, build their self-confidence, make new friends, and feel a sense of camaraderie and support.

The costs of playing organized hockey have typically meant that many families have been unable to afford the sport. The goal of the Hockey4Everyone program is to give economically disadvantaged children the opportunity to play House league hockey at no or only a minimal cost to themselves.2

For the 2011-2012 hockey season, Forest Hill Hockey Association (FHHA) has partnered with Canadian Tire Jumpstart Charities (Jumpstart) to sponsor up to 50 economically disadvantaged children to play House league hockey. For each approved applicant, FHHA will pay the registration fee3 (up to a maximum of $500) for the upcoming 2011-2012 season at a House4 league in the Greater Toronto Area and Jumpstart will provide a certificate for the purchase of new hockey equipment5 up to a maximum value of $300 at a local Canadian Tire retail store.

Children between the ages of 6 and 16 will be selected according to need, determined exclusively by income. FHHA and Jumpstart will rely on Statistics Canada's Low Income Cut-Offs (LICO), which designate an income threshold below which a family is likely to spend significantly more of its income on food, shelter and clothing than the average family spends (see below for thresholds).

If your total before-tax household income falls under the threshold listed above for your family size, please send in an application for each of your children ages 6 through 16 who are interested in playing hockey.

The completed application form, with proof of income (a Notice of Assessment or Reassessment issued by the Canada Revenue Agency for the 2010 tax year for every member of the household with income), can be submitted either by email or mail.

Incomplete applications and applications that do not include proof of income cannot be considered.

Decisions will be made on a first-come, first-served basis as qualified applications are received until all spaces have been filled. In no case, however, will applications be considered after September 1, 2011.

NOTES:

1Hockey4Everyone is the continuation of 50 for 50! which was launched in 2010 in celebration of Forest Hill Hockey Association's 50th anniversary.

2There are per-child funding limits of $500 for the registration fee and $300 for equipment. The applicant's family will be responsible for any and all amounts/costs in excess of these limits.

3FHHA will pay the registration fee (up to a maximum of $500) to the selected hockey association/club on the child's behalf.

4House leagues are less competitive, entry-level, non-contact leagues. They are typically the type of league that a beginner would join, although they will include a mix of beginners and more advanced players. Registration fees will not be paid for any other type of league, including the more competitive Select or Minor leagues.

5Canadian Tire Jumpstart certificates will not be issued until after a child has been successfully registered with a hockey club/association.
